You will receive a copy of this form via email. You will also be notified when your grant has been forwarded to the district office (please allow at least two weeks for this).

Purpose

The purpose of this form is to streamline the grants process and serve as a guide for what paperwork needs to be submitted with grants. This form will also collect information about any data reporting requirements associated with grants so that the PRIE office can be better prepared to assist in providing the data.
